Exchange 2010 New Mail Notifications Not Working in Outlook Online Mode

We have just migrated from Exchange 2007 to Exchange 2010 (the 2007 servers are still around with a handful of mailboxes). At this primary site we have two Exchange servers with all three roles - HT, MB, CAS. These servers are behind a load balancer. We have a CAS array defined with these two servers which is the CAS server for all mailboxes at this site.

After customers were moved to Exchange 2010, new mail notifications worked intermittently for people in 'online mode' (the Use Cached Exchange mode box was UNCHECKED). The new mail notifications would work for awhile (a few hours), then would stop working. They do not get an envelope icon in their desktop nor do they get a pop up preview of the message. Everyone who is in 'cached' mode gets notifications successfully.

When I change someone from online to cached mode the notifications work again permanently. Or I can just close and re-open Outlook and the notifications will work again for a bit, then not.

Any ideas on what steps I do to troubleshoot where my issue is? I posted this here in Exchange forum because the notifications were working fine for customers on 2007. Outlook clients are Outllook 2003 SP3 (11.8330.8341) Thanks in advance for the help.
